Course Outline

In general, workplace injuries not only cause suffering and great financial loss to workers, but also negatively impact their families, their employers, and society as a whole. Many more “near misses” or “close calls” also happen. These are incidents that could have caused serious injury or illness but did not, often by sheer luck. Practically all of these harmful incidents and close calls are preventable, if previously studied. This course describes an OSHA-endorsed 4-step process that investigators should follow, provides a list of questions that can be used to identify the root cause of an incident/accident, and discusses the essential elements of an investigative report. In addition, an illustrative investigation report is included in the course material.
